CVS Health & Amazon
CVS Health and Amazon Pharmacy are direct competitors in the US retail pharmacy market, with Amazon rapidly expanding its prescription drug delivery capabilities. Amazon Pharmacy achieved same-day delivery for prescriptions in major metro areas by late 2025 and plans to build new pharmacy operations every 18 days to reach 45% of US customers with same-day delivery by end of 2025. CVS operates 9,000+ retail pharmacy locations and the Caremark PBM, creating a multi-channel business model that Amazon's pharmacy-only approach cannot currently replicate. Amazon's Eli Lilly dispensing partnership makes it a preferred GLP-1 access point, directly threatening CVS's pharmacy share in the fastest-growing drug category.
Why it matters: CVS's vertical integration spanning retail pharmacies, Caremark PBM (the largest PBM in the US), and Aetna health insurance creates an ecosystem that Amazon cannot easily replicate. However, Amazon's logistics superiority, Prime loyalty program (200M+ members), and consumer willingness to order medications online pose an existential threat to CVS's retail pharmacy revenue. CVS's pivot to health hubs and MinuteClinic in-store health services attempts to create physical touchpoints that Amazon cannot replace with delivery.
